Resource Lending Library and Van Schedule

Before borrowing materials from the Resource Lending Library (RLL), the following information is needed

 

  1. Copy of current facility license from Community Care Licensing

  2. Signed “Instructions For Care” form (given by RLL staff at the time of first visit)

  3. Completed “Signature Card” (given by RLL staff at the time of first visit)
